Reminder to activate CS accounts

Form Fields:
1) Checkbox - Use checkbox to select players. (you can select any/all players)
2) E-mail Id - The team member's email account defaults. Please review the email id or enter the correct email id to which the reminder with account details needs to be sent
3) Comments - Enter comments if any (optional)
Action:
Send - Click send button to send the emails to selected players.
Close - Click Close to close the form.
